You must make application to the City in order to receive these points by returning the attached form. Application must be made to my office at

800 N. Field Drive, Lake Forest, IL 60045 within 10 days of the date of this email (August 2, 2019) and appropriate proof must accompany your request.

 

If you have already provided some of the documentation with your supplemental application, please include another copy. Forms received without the appropriate documentation attached will be rejected and the preference points not assigned.
